Management of Benign Pigmented Lesions

Intense Pulsed Light (IPL) is a non-invasive medical treatment used to manage benign pigmented lesions on the face and body. These include solar lentigines (often called age or liver spots) and ephelides (freckles), which are typically associated with the physiological ageing process and chronic sun exposure.

While these lesions generally pose no health risk, they are a clinical sign of photodamage. It is important to note that not all pigmentation is suitable for this treatment; hormonal pigmentation, such as melasma, requires a specific diagnosis and a different management approach.

For suitable conditions, a treatment course of one to three sessions may be recommended. A full medical consultation and test patch are mandatory before commencing treatment.

For pigmented lesions, specific wavelengths of light are absorbed by melanin (the pigment within the lesion). Pre-Treatment Consultation

Your treatment begins with a full medical consultation to establish a clinical diagnosis and confirm your suitability. This includes a review of your medical history and a mandatory test patch.

The IPL Treatment

Your practitioner uses a hand-held device to deliver controlled pulses of light to the diagnosed lesions. The procedure duration is approximately 30 minutes. Patients may feel a sensation similar to a brief flick of an elastic band.

Post-Treatment Care and Expectations

Immediate Aftercare

Post-treatment, the skin may appear red or slightly swollen, but these effects usually subside within a few hours. Avoiding sun exposure and using a high-SPF sunscreen to protect the treated area is very important. Following the practitioner’s aftercare instructions is essential for optimal results and skin health.

Long-Term Results and Maintenance

IPL treatments can significantly improve skin pigmentation, with results visible after one to three sessions. However, maintenance treatments may be necessary to preserve these results, especially if new pigmentation issues arise. A personalised treatment plan will be discussed during your consultation.

It’s important to note that IPL may not be suitable for those with certain medical conditions or skin types, which will be determined during the pre-treatment consultation.

FAQs For Pigmentation Treatment

Light is released in short pulses that are readily absorbed by the high concentration of melanin found within pigmented lesions. The rapid absorption of light energy heats the melanin and causes the destruction of the melanin-rich cells.

Most people are suitable for treatment except for very dark skinned or tanned individuals. However, large, dark, mottled or raised pigmentation cannot be treated and may need to be checked by a dermatologist. Large, dark moles should not be treated, nor should lesions covering a large part of the body. The pigmented marks that respond the best are superficial lesions such as those caused by sun damage (sun spots) and freckles.

Most patients describe the discomfort as mild and tolerable and no anaesthesia is required.

The treated area may feel warm and sensitive for the first few days post treatment. The lesions may darken and may appear more obvious before they fade and ‘flake’ away. Sometimes an initial whitening of the area is seen, which quickly fades and the pigment gradually fades over the following few weeks.

The skin that surrounds the pigmented lesion may become red immediately after treatment. Most people experience no other side effects and the redness usually disappears within hours to a few days. Very rarely a small blister may form or the skin may become temporarily lighter or darker.

The light applicator is placed on the skin and a short pulse of light is released. The applicator is then moved to the neighbouring area and the process is repeated until the entire area is treated. With Q-Switched Laser treatments due to the power of the Laser energy delivered into the skin, a small amount of pinpoint bleeding may occur during the treatment, and a dressing may be applied after the treatment to protect the skin while it heals. During treatment protective eye wear will be provided.

The number of treatments will depend on the type and size of the pigmented lesion but typically 1 to 3 sessions are required at 4 to 6 week intervals
