Preventative Treatment for Chronic Migraine

Chronic migraine is defined as experiencing headaches on 15 or more days per month over a period longer than 3 months, with at least 8 of these days having migraine features. The frequency and severity of these headaches may be substantially affecting your daily functioning and overall quality of life. 

This page explains neurotoxin (specify Botulinum toxin type A (e.g. “commonly known as Botox®”)) injections as a preventative treatment option available at Rejuvenating Solutions, intended to reduce the frequency of diagnosed chronic migraines. This treatment does not stop a migraine already in progress.

Understanding Chronic Migraines

Chronic migraine is characterised by frequent headaches. To meet the definition, you must experience headaches on 15 or more days per month for longer than 3 months, with at least 8 of those headache days showing migraine features. Migraine symptoms often include –

  • Severe, throbbing pain, frequently on one side of the head
  • Nausea or vomiting
  • Increased sensitivity to light and sound

Neurotoxin for Chronic Migraine Prevention

Neurotoxin injections are an approved preventative treatment for adults who have been diagnosed with chronic migraine.

Mechanism – It is believed that the neurotoxin interrupts pain signalling pathways involved in the development of a migraine headache.

Treatment Protocol – The treatment involves injecting defined neurotoxin units into specific, shallow muscles following a set pattern (based on the PREEMPT study) across the forehead, temples, back of the head, neck, and upper back/shoulders.

Frequency – To maintain its preventative effect, treatment is typically administered by Susie every 12 weeks.

Goal – The aim is to reduce the number of headache days you experience each month and lessen the severity of migraines. For some patients, 2 or 3 treatment cycles may be needed to notice a significant reduction.

NICE* recommends botulinum toxin type A for adults with chronic migraine who have failed to respond to at least three different oral preventive treatments and whose medication overuse has been appropriately managed.

Your Migraine Treatment Consultation

During your consultation appointment, Susie will carefully review your migraine history. This includes the pattern and frequency of your headaches and any previous treatments attempted. 

Susie will assess your suitability for this preventative neurotoxin treatment based on established clinical guidelines. She will explain the specific injection procedure, potential benefits, possible side effects (including common but usually mild ones like neck pain, and less common ones like temporary eyelid drooping), and the treatment cost. This is your opportunity to ask questions and ensure you fully understand your proposed plan.

The Treatment Procedure

The neurotoxin treatment for chronic migraine prevention is administered by Susie according to the specific PREEMPT protocol*.

Using a very fine needle, Susie will inject small amounts of the neurotoxin solution into the predetermined muscle sites around your head and neck. The appointment for the injections usually takes approximately 15 to 20 minutes. Most patients tolerate the procedure well, reporting only mild, temporary discomfort during the injections.

Aftercare and Considerations

Minimal downtime is expected after chronic migraine treatment. You can typically resume normal activities shortly after your appointment.

Potential side effects often include temporary neck pain or stiffness, or a headache following the procedure. Less commonly, temporary weakness in nearby muscles, such as eyelid drooping, can occur but usually resolves on its own. Susie will discuss these possibilities during your consultation. Adherence to the 12-week treatment schedule is important for maintaining the preventative effect.
